The app provides a missing Contact Us form for the Office 365 website. Visitors fill in their contact information to be stored in SharePoint. App provides standard list of fields with customizable visability and requirement settings:

First Name;

Last Name;

Company;

Job Title;

E-Mail;

Phone;

Address;

City;

State/Province;

ZIP/Postal Code;

Country/Region

Comments;

Admin has access to the list of submissions and can easily modify form by adding or removing fields. App provides the following features:

Works with public SharePoint sites with anonymous access.

Standard SharePoint email alerts for the list of submissions.

App part can be reused on multiple pages of the site with unique settings and a way to destinguish between submissions from different forms.
